Histological Analysis of Breast Tumous



Abstract
From September 1983 to August 1991, 795 patient including 19 males of gynecomastia had surgically treated with breast masses. All tumors were histologically classified according to the WHO classification of the breast tumors. 80.3% were benign
and
all
lesions were confirmed by excisional biopsy. Malignancy was identified in a total of 157 patient(19.7%); 58 patients had a diagnostic excisional biopsy but 99 patients had one-stage operation with establishing a definitive diagnosis by frozen
section or
previous confirmation at the different clinics. About 8% of patients undergoing a diagnostic excisional biopsy had malignancy. The most common benign lesions were fibroadenoma(39.2%) and mammary dysplasia(40.1%) with a peak age of 20' and 30',
respectively, followed by inflammatory pseudotumor, gynecomastia, lipoma, adenoma, intraductal papilloma, cystosarcoma phylloides, duct ectasia, and cysticercosis in order of frequency. In 73 inflammatory pseudotumors, most of pathologic results
revealed chronic granulomatous inflammation except 6 lesions of fat necrosis, 6 of tuberculous mastitis, 3 of radial scar, and 2 of paraffinoma. 92% of the malignant lesions were invasive and most of them were ductal type. Only four patients had
noninvasive lesion. WE conclude that three most common pathologic conditions of the breast were fibroadenoma, mammary dysplasia, and cancer. The relative age specific incidence of malignant disorders increased after 5th decade. Mammary masses in
the
young female should be also considered for excision because this country is still prevalent for tuberculous mastitis or cysticercosis. Very low incidence of noninvasive carcinoma(2.5%) warrants us a need for an effort to diagnose cancer at a
earlier
stage.
